As a parent/guardian of the above named child/children, I hereby release Park East Day School (“the school”), its agents, employees and trustees from all liability arising from his/her transportation via school bus or other vehicle to or from the school. As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the above named student. I agree on behalf of myself, my child named herein, or our heirs, successors and assigns, to hold faultless and defend Park East Day School, its officers, directors and agents from any and all actions, claims, demands, damages, costs, expenses and all consequential damage arising from or in connection therewith, and I agree to compensate the school, its officers, directors and agents for reasonable attorney’s fees and expenses arising therewith. I understand that it is my full responsibility as parent/guardian to (1) Accompany the child to the bus/vehicle pickup location in the morning, and to pick up the child in the evening at the drop off location. (2) Instruct my child/children as to his/her pickup and drop off point. (3) Review with my child/children the transportation rules provided by the school.
